ENTERING OUR PROGRAMS

Admission into programs and services of Heartland Opportunity Center is based on the individual needs of the applicant, as determined by referral information, and on the ability of the organization to meet the applicant's needs.

Individuals are considered for services without regard to race, color, sex, religion, national origin, age, political affiliation, physical or mental disabilities.

Heartland promotes full accessibility for all the persons it serves. If it is determined that services cannot be provided, a referral to an appropriate program will be made.

Individuals enter Heartland through a professional referral agency such as Central Valley Regional Center or Department of Rehabilitation.

Referrals for programs and services should be directed to the program or service requested.

GENERAL CRITERIA FOR ADMISSION

All applicants must meet the following entry criteria:

  • age 16 or older;
  • diagnosed mental, physical or emotional disability;
  • current (within past year) medical report;
  • TB test result;
  • psychological report;
  • controlled medical/physical condition not requiring extensive intervention;
  • ability to care for personal needs or provision of attendant to do so;
  • reasonable expectation that applicant will benefit from program;
  • consent of applicant or responsible party to admission;
  • behavior which would not significantly interfere with progress of others;
  • behavior which would not be harmful to self or others;
  • source of funding.
